Premium Care Wellness Checks · Effective September 25, 2026 · Updated October 22, 2026
This policy explains what information Premiumcare, LLC (“we”, “us”), collects through premiumcarewellnesscheck.com and the Premium Care Wellness Checks phone service. It also covers how we use that information and the choices you have.
1. What we collect
- About you, the subscriber: your name, email, phone number, billing address, and plan. Card payments are handled by Stripe. We never receive or store your full card number.
- About the person we call: their name and preferred name, phone numbers, home address, date of birth, language, who lives with them, and any pets in the home. We also collect the call times you choose, the reminders you ask us to give (for example, medication reminders), and any notes you share to help our callers.
- For residents of a senior community: the community gives us the resident’s name, room, phone number, preferred call time and the loved ones to notify. If someone decides for the resident (a POA or responsible party), we also receive that representative’s name and contact details. We record each resident’s or representative’s yes or no to the calls and notes.
- About your contacts: the names, relationships, emails, and mobile numbers of the family members, neighbors, or key holders you list.
- Call records: the time of each call attempt, whether it was answered, and the note our caller writes afterward.
- Your authorization and signature: the escalation steps you authorize, your typed signature, the date, and your IP address.
- Website data: basic technical information such as browser type and pages visited. This may be collected through cookies or analytics tools.
2. How we use it
- To place the calls, give the reminders you asked for, and send notes to your contacts.
- To follow the missed-call plan you authorized, including giving the home address to responders when you have told us to.
- To bill you, send receipts and renewal reminders, and answer your questions.
- To supervise and improve the quality of our service, and to meet our legal obligations.
3. Who we share it with
- The contacts you list: they receive call notes and missed-call alerts.
- Our team: the RN-supervised Care Callers who make the calls and the staff who manage our care operation. Some callers work remotely and may be located outside the United States; they access only what they need to make and log your calls, under our instructions. Details such as date of birth, doctor and full address are limited to our owner unless they are needed to get someone help.
- The resident’s community: on community plans, the community’s staff receive missed-call alerts and manage which residents are enrolled.
- Service providers who work for us: payment processing (Stripe), and email, text-message, phone, and website hosting providers. They may use the information only to provide their service to us.
- Responders, on your instruction: local police or a key holder, when you have authorized that step. We also contact 911 if someone may be hurt or in danger.
- When the law requires it: for example, in response to a valid legal request, or to report suspected elder abuse as the law requires.
We do not sell personal information, and we do not share it for targeted advertising. We will not share your mobile number or text-message consent with any third party for their marketing.
4. Text messages and calls
If you choose text updates (at checkout or on our Text Updates page), we use your mobile number only to send messages about your loved one’s calls: call notes and missed-call alerts. Texts come from our toll-free number (888) 592-6359. Message frequency varies, up to 4 messages per day. Message and data rates may apply. Reply STOP at any time to stop, or HELP for help. Consent to texts is not a condition of purchase.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text-message opt-in data and consent are excluded from all of the sharing described above and will not be shared with any third party, except the text-message provider that delivers our messages on our behalf.
Our calls are placed from our own business phone numbers through our phone provider (Twilio). We do not record calls.
5. How long we keep it
We keep your account and call records while your plan is active. After you cancel, we keep them for as long as we need them for legal and tax purposes, and then we delete them.
6. How we protect it
Access is limited to the staff who need the information to provide the service. We use service providers with industry-standard security. No system is perfectly secure, but we take reasonable steps to protect what you give us.
7. Your choices and rights
You can ask us to show you, correct, or delete the information we hold about you or your loved one. You can also change your contacts or text preferences at any time. California residents have these rights under state law, and we extend them to all our subscribers. We will not treat you differently for using them. To make a request, call (951) 405-3134 or email info@premiumcarewellnesscheck.com. We may need to confirm your identity first. You can stop the calls yourself at any time at premiumcarewellnesscheck.com/stop-calls.
8. Children
This service is for adults. We do not knowingly collect information from children under 13.
9. Changes
If we change this policy in a way that matters, we’ll post the new version here and email active subscribers.
